 PART I: Information Sheet


I am MJ Moradian and I am a PhD candidate at Tehran University of Medical Sciences.

Risk prone countries like Iran should be prepared for disasters as much as possible. One the major surges in first phase of an event like earthquake is influx of injured peoples to hospitals and one of the solutions is summoning field hospitals to the scene.

Field hospital is a mobile, self-contained, self sufficient health care facility capable of rapid deployment and expansion or contraction to meet immediate emergency requirements for a specified period of time. The base of FH could be tent, container, conex, bus or a mixture of them.

Assembling and maintaining a field hospital is expensive so finding the real numbers of needed FHs and their locations in the country could be a persecutor.

As a thesis project I am going to find out how many “field hospital” does a country need during the preparedness for future disasters and which locations are better to be chosen to stand by the field hospital there to have a good response time when they needed.

The cities are candidate for storing FHs because they have enough logistic support to summon them with educated staff during a crisis.



We listed all related criteria that can help us to weight the candidate cities in order to assign a field hospital for them to maintain that.

This consent is for “Delphi” study to confirm the list of criteria for this site selection and also weighting them.

Please do not hesitate to ask any further question about the project.

Type of Research Intervention

I will send a few E-mails which is a simple questionnaire that in a few minutes you can answer its questions.

Selection of Participants

Researchers and experts who worked on hospital site selection projects and/or deal with field hospital can participate in this study. So a list of authors of related articles is prepared to send them this consent.


Voluntary Participation

Participation in this study is voluntary. You can ask as many questions as you like and we take the time to answer them.



You will fill out a questionnaire which will be provided by E-mail. If you do not wish to answer some of the questions included in the questionnaire, you may skip them and move on to the next question. The information recorded is confidential, and no one else except MJ Moradian will have access to the questionnaire. The questionnaires will be deleted after the publication of results.



Filling the questionnaire takes only a few minutes (10 to 15). As it is a Delphi method for consensus so you may receive about 3 E-mails.


Risks and Discomforts

We are not asking you to share with us personal and confidential information more than name, address, contact information and sum of years of your experience about the subject. If you are uncomfortable about any of the questions you can simply do not answer that. However, we do not wish for this to happen.



There will be no immediate and direct benefit to you, but your participation is likely to help us find out more about the health necessities in disasters and we hope that these will help governance and other international agencies in helping peoples in emergencies.



 You will not be provided with any payment to take part in the research.


We will not share your information outside of the research team. The information that we collect from this research project will be kept confidential. Any information about you will have a number on it instead of your name. Only the researchers will know what your number is and we will put a private password for the file of information. No hard and soft copy of named information will remain after the study will be finished. It will not be shared with or given to anyone except the research team.


Sharing of Research Findings

At the end of the study, we will be sharing what we have learnt with the participants and with the community. We will do this by sending E-mail to participants and publish the results in order that other interested people may learn from our research. Nothing will be attributed to you by name.


Right to refuse or withdraw

You may choose not to participate in this study if you do not wish to do so. You may stop participating in the Delphi at any time that you wish without either of you losing any of your rights here.

I have been asked to give consent to participate in this research study which will involve me completing one questionnaire. I have read the foregoing information. I have had the opportunity to ask questions about it and any questions that I have asked have been answered to my satisfaction.  I consent voluntarily to participate in this study.

Statement by the researcher

I have sent the information sheet to the potential participant, and to the best of my ability made sure that the person understands that the following will be done:

1. Three or four E-mails will be sent to the participants which is containing a questionnaire about field hospital site selection.

I confirm that the potential participant were given an opportunity to ask questions about the study, and all the questions asked by them have been answered correctly and to the best of my ability. I confirm that the individual has not been coerced into giving consent, and the consent has been given freely and voluntarily.
